What is iron?
Iron is a mineral naturally present in the human body that plays a central role in countless bodily processes. Its best-known function is its involvement in the production of hemoglobin. Hemoglobin is a protein in red blood cells that absorbs oxygen in the lungs and transports it to all tissues and organs in the body.
In addition to its role in blood formation, iron is also part of various enzymes and proteins involved in energy production and the immune system. Your body cannot produce iron itself, so you are entirely dependent on what you ingest through your diet. Good dietary sources include red meat, legumes, dark green leafy vegetables, and whole grains.
A healthy iron balance is essential. Both a deficiency and an excess can have noticeable consequences for how you feel and function on a daily basis.
Why is iron measured?
A blood test for iron provides insight into how much free iron is circulating in your blood. This is often combined with additional values, such as ferritin (iron storage in your body) and transferrin (the transport protein for iron), to get a complete picture of your iron status.
Doctors measure iron levels when there are symptoms consistent with anemia, such as prolonged fatigue, concentration problems, or a pale complexion. Periodic monitoring is also beneficial for people at increased risk of iron deficiency, such as pregnant women, people with a one-sided diet, or individuals with chronic bowel problems.
An iron measurement may also be requested if iron overload is suspected, a condition in which the body absorbs too much iron and stores it in organs.
What do abnormal values mean?
Low iron level
A reduced iron level in the blood can indicate an iron deficiency or iron-deficiency anemia, also known as iron-deficiency anemia. In this form of anemia, the body has insufficient iron to produce enough hemoglobin. The result is that the tissues receive less oxygen than they need.
Symptoms that may accompany this include extreme fatigue, reduced concentration, dizziness, shortness of breath during light exertion, palpitations, headaches, and sometimes a ringing in the ears. The severity of the symptoms depends on how far the deficiency has progressed. When anemia develops gradually, the body sometimes adapts, causing a person to experience relatively few symptoms despite a significant deficiency.
A low iron level can also result from a chronic inflammatory response in the body, where iron is present but not readily available for blood formation. This is called functional iron deficiency.
High iron levels
An elevated iron level can indicate an excess of iron in the body. A known cause of this is hemorrhomatosis, a hereditary condition in which the intestines absorb too much iron from food. Excess iron accumulates in organs such as the liver, heart, and pancreas, which can lead to organ damage in the long term.
Additionally, a high iron level can also indicate acute liver damage, where iron is released from damaged liver cells. It is always important to interpret an abnormal value in the correct context, together with other blood values and your symptom pattern.
When is it useful to have your iron levels measured?
Measuring your iron level is useful if you have symptoms that could be consistent with an iron imbalance. Examples include persistent fatigue that cannot be easily explained, recurring headaches, difficulty concentrating, or noticeably pale skin. It is also advisable to check your iron status in case of regular blood loss, for example due to heavy periods.
People at increased risk of iron deficiency, such as vegans, vegetarians, athletes with a high training load, or people with a gastrointestinal disease that impairs nutrient absorption, may benefit from periodic monitoring of their iron levels.
