Urea is a waste product released during the breakdown of proteins. The amount of urea in your blood says a lot about how well your kidneys are working.

What is urea?
Urea is a waste product that your body produces during the processing of proteins and amino acids from food. When cells break down proteins, amino acids are released. The nitrogen from those amino acids is converted into ammonia, a toxic substance. To neutralize this, the liver processes the ammonia via the urea cycle into urea, a much less harmful compound.
The urea produced in this way enters the bloodstream and is subsequently filtered by the kidneys and excreted in the urine. The rate at which this happens is closely related to how well your kidneys function. This makes urea a useful indicator of kidney health.
Urea is also referred to as blood urea or BUN (Blood Urea Nitrogen). In a blood test, the concentration of urea in the serum or plasma is determined, expressed in millimoles per liter (mmol/L).
Why is urea measured?
Doctors measure urea primarily to get an impression of kidney function. The kidneys are responsible for filtering waste products from the blood. If they do this less effectively, urea accumulates in the bloodstream and the measured value rises.
Urea is rarely viewed as a single value. It is almost always combined with creatinine, another waste product excreted by the kidneys. The ratio between urea and creatinine provides additional information about the cause of any potential abnormality. Together, these markers help the doctor form a more targeted picture.
In addition to assessing kidney function, urea is also relevant for monitoring protein metabolism. In people following a high-protein diet, breaking down significant muscle mass, or suffering from certain conditions, the urea level can be a reflection of this.
What do abnormal values mean?
Urea level too high
An elevated urea level in the blood is called hyperuremia. This can be an indication that the kidneys are less able to filter waste products, such as in cases of kidney damage or renal insufficiency. The higher the value, the greater the likelihood that there is an issue with kidney function.
Not every increase directly points to kidney disease. Dehydration is a common cause: if there is insufficient fluid in the body, urea becomes more concentrated in the blood. Bleeding in the gastrointestinal tract can also increase the urea level, because blood is broken down and the proteins present in it produce extra urea. Additionally, increased protein breakdown, for example due to intensive physical exertion or certain medical conditions, can temporarily cause the value to rise.
A single elevated value is therefore not immediately alarming. Nevertheless, it is advisable to always discuss abnormal results with a doctor so that the context and any subsequent steps become clear.
Urea value too low
A urea value that is too low occurs less frequently and therefore attracts less attention, but it can also be significant. A possible cause is severely reduced protein intake, for example due to malnutrition or a one-sided diet. The liver then simply produces less urea.
Liver problems can also lower the urea value. After all, the liver is responsible for the production of urea via the urea cycle. If liver function deteriorates, less urea can be produced, which translates into a lower blood value. A low urea value combined with other abnormal liver values therefore warrants further investigation.
When is measuring useful?
A urea determination is part of many standard blood tests and health check-ups. It is a relatively simple and affordable test that provides valuable information about kidney function and protein metabolism.
Measuring is certainly useful if you have symptoms that may indicate kidney problems, such as fatigue, reduced urine production, swollen ankles, or high blood pressure. It is also advisable to have kidney function checked regularly, including urea, in cases of diabetes, high blood pressure, or a family history of kidney disorders.
People who take medication that strains the kidneys, or who are recovering from a serious illness, can benefit from periodic measurements. In this way, the urea value over time provides a reliable picture of how the kidneys are performing and whether a treatment is effective.
Finally, urea is also useful as a reference point during a general health check. In combination with other biomarkers, it provides a complete picture of how organs such as the kidneys and liver function, without the need for extensive symptoms.
