Thyroxine, also known as T4, is a hormone produced by the thyroid gland. It plays a central role in regulating your metabolism and helps your body maintain the correct temperature. However, not all thyroxine in your blood is actually available to your body cells.
A large portion of the T4 in your blood is bound to proteins. The body cannot use this bound form. Free T4 is the portion that is not bound to proteins. It is precisely this free portion that is active and can reach the cells where it performs its function. For this reason, free T4 provides a more reliable picture of your thyroid function than a measurement of total T4.
The thyroid gland is located at the front of your neck and produces T4 under the control of TSH, the thyroid-stimulating hormone produced by the pituitary gland. Free T4 and TSH are therefore often measured together to obtain a complete picture.
Why is Free T4 measured?
A measurement of free T4 provides insight into the actual amount of thyroid hormone your body has available. This is useful for detecting and confirming thyroid disorders, but also for monitoring ongoing treatment.
Suppose your TSH value is elevated, which can indicate an underactive thyroid. If free T4 is nevertheless normal, this may indicate that the problem does not lie in the thyroid itself, but possibly higher up in the chain, namely in the pituitary gland or the hypothalamus. This nuance is medically important, because the cause determines the treatment.
Free T4 is also a valuable measure for people taking medication for a thyroid disorder. It allows a doctor to check whether the dosage is properly adjusted and whether the treatment is having the intended effect. Free T4 then provides a concrete answer to the question: does the body have sufficient active thyroxine available?
What do abnormal values mean?
Value too low: possible hypothyroidism
When free T4 is lower than the reference value, this may indicate hypothyroidism. With this condition, the thyroid gland produces too little thyroxine. Metabolism slows down, which can manifest as symptoms such as fatigue, feeling cold, weight gain, and a slow heart rate. Hypothyroidism is highly treatable, usually with the daily intake of synthetic thyroid hormone.
A low free T4 value does not always mean that the thyroid gland itself is the problem. Sometimes the cause lies in the pituitary gland, which produces insufficient TSH to stimulate the thyroid gland to produce it. A doctor always assesses the free T4 value in conjunction with other hormone levels to draw the correct conclusion.
High value: possible hyperthyroidism
An elevated free T4 value can indicate hyperthyroidism, in which the thyroid gland is overactive and produces too much thyroxine. The metabolism then runs at too high a rate. Symptoms that may accompany this include palpitations, unintentional weight loss, heat intolerance, irritability, and increased sweating.
A free T4 value that is too high also requires further medical evaluation. Treatment depends on the underlying cause and may consist of medication, radioactive iodine, or, in some cases, surgery.
When is measuring useful? There are various situations in which a measurement of free T4 provides valuable information. If you have symptoms that could be consistent with a thyroid problem, a blood test is a logical first step. Think of persistent fatigue without a clear cause, unexplained weight changes, or a feeling that your heartbeat is off.
Free T4 is also measured in addition to a TSH measurement that yielded an abnormal result. Together, they provide a clearer picture than either individually. Pregnancy is another situation in which thyroid function is closely monitored, because thyroxine plays a role in the development of the fetus.
If you are already taking medication for a thyroid condition, periodic measurement of free T4 helps to properly tailor the treatment. It is an objective way to assess whether the current dosage matches what your body needs.
