What is alkaline phosphatase?

Alkaline phosphatase, abbreviated ALP, is an enzyme that occurs naturally in the body. Enzymes are proteins that accelerate chemical reactions. ALP specifically cleaves phosphate groups from other molecules and does so best in a basic (alkaline) environment. Hence the name.

The enzyme is present in various tissues: the liver, the bile ducts, the bones, the small intestine, the kidneys, and the placenta during pregnancy. In adults, the vast majority of ALP in the blood originates from the liver and the bile ducts. This makes it a logical marker in the screening for liver and bile duct disorders.

Because ALP is also active in bone tissue, osteoblasts, the cells that create new bone tissue, also produce ALP. Consequently, the ALP level in the blood rises measurably with increased bone remodeling.

Why is ALP measured?

ALP is part of the standard liver profile, a series of blood values ​​that together provide an overview of how the liver functions. The enzyme helps distinguish between different types of liver problems. For instance, a significantly elevated ALP indicates a disorder in bile flow rather than direct damage to liver cells.

In addition to liver examinations, the doctor may order an ALP test if bone disorders are suspected. In diseases where bone tissue is broken down or formed at an accelerated rate, the ALP level rises. It is therefore a useful addition to other bone markers.

ALP is also relevant during pregnancy. The placenta produces its own form of the enzyme, causing ALP to rise naturally in the third trimester. This is a normal physiological adaptation and no cause for concern.

What do abnormal values ​​mean?

Elevated ALP

An ALP value that is too high has various causes. In the liver and bile ducts, an elevated level may indicate cholestasis, an obstruction of bile flow. This can be the result of gallstones, but also of primary biliary cholangitis, a chronic autoimmune disease of the bile ducts. Liver cirrhosis, hepatitis, and liver metastases can also affect ALP levels.

From the skeleton, there are also various possible causes. Paget's disease, in which bone remodeling is severely disrupted, leads to very high ALP levels. Rickets and osteomalacia, both caused by vitamin D deficiency, are associated with reduced bone mineralization and can increase ALP as a compensatory mechanism. Bone metastases, in which cancer spreads to the bones, increase bone remodeling and thus ALP. Hyperparathyroidism, an overactivity of the parathyroid glands, stimulates bone breakdown and can similarly increase ALP.

Less well-known causes include infections, inflammatory bowel diseases, or certain medications that affect the liver or bone metabolism. An elevated ALP is therefore a signal that warrants further investigation, but it does not constitute a diagnosis in itself.

Decreased ALP

A low ALP level occurs less frequently and is not always considered problematic. Nevertheless, there are situations where low values ​​warrant attention.

A rare genetic disorder, hypostatasis, causes reduced ALP production, resulting in problems with bone formation and mineralization. The severity varies widely: some adults experience few symptoms of the condition, while severe forms in children lead to bone problems early in life.

Nutritional deficiencies can also lower ALP. Zinc and magnesium are both necessary for normal enzyme function, and a deficiency in these minerals can reduce ALP activity. Protein deficiency due to malnutrition can also inhibit enzyme production. Furthermore, thyroid abnormalities such as hypothyroidism and conditions involving overproduction of cortisol, such as Cushing's syndrome, can influence ALP levels.

When is an ALP measurement useful?

An ALP determination is useful if there are symptoms that may be consistent with liver or bile duct problems, such as jaundice, pain in the upper right abdomen, fatigue, or dark urine. The doctor may also decide to include an ALP test in cases of bone pain, bone deformities, or suspected vitamin D deficiency.

In addition, ALP is a standard part of broader health screenings. In combination with other liver values, such as ASAT, ALAT, and GGT, it provides a more complete picture of liver health. It is not possible to make a diagnosis based on ALP alone. The value gains meaning in conjunction with symptoms, other laboratory values, and any imaging studies.
