The name 'granulocytes' refers to the granular structure visible when viewing these cells under a microscope. These granules contain enzymes and proteins that allow the cell to break down invaders. Neutrophils react quickly: as soon as an infection or damage occurs anywhere in the body, they move to the site of the problem.

Because neutrophils are so central to the first line of defense of our immune system, their number in the blood provides valuable information about how the body responds to disease, stress, or other disturbances.

What is it measured for?

Measuring neutrophil granulocytes is performed as part of a differential blood count, also known as white blood cell differentiation. This test determines not only the total number of leukocytes but also the distribution across the different subtypes: neutrophils, lymphocytes, monocytes, eosinophils, and basophils. Each cell type has its own function, and a shift in that distribution can guide a diagnosis.

The measurement is typically performed via automated analysis of a blood sample. What do abnormal values ​​mean?

Low neutrophil count

A reduced number of neutrophils is called neutropenia. This can indicate a severe infection in which the body consumes so many neutrophils that production cannot keep up with the demand. However, conditions that affect bone marrow function, such as certain blood diseases, can also be responsible for low values.

In addition to disease processes, external factors also play a role. Some medications, including specific antibiotics and immunosuppressants, can inhibit the production of neutrophils. A deficiency in vitamin B12 or folic acid disrupts the normal maturation of blood cells in the bone marrow. Excessive alcohol consumption can also contribute to reduced production.

A low neutrophil count always warrants attention, as it increases vulnerability to infections. People with neutropenia are at greater risk of serious or long-lasting infections that would resolve on their own in others.

Excessively high neutrophil count

A doctor most often sees an elevated number of neutrophils, or neutrophilia, in bacterial infections or inflammatory reactions somewhere in the body. The immune system responds to the threat by producing more neutrophils and sending them into the blood.

Illness is not the only factor that can raise the values. Short-term physical exertion, stress, or the use of corticosteroids (such as prednisone) can temporarily cause higher neutrophil counts. In those cases, the increase is usually transient and not cause for concern. If persistently high values ​​remain without a clear explanation, further investigation is warranted.

When is it useful to have neutrophils measured?

A measurement of neutrophil granulocytes is useful if you repeatedly become ill, suffer from prolonged fatigue, or if your doctor suspects that your immune system is not functioning optimally. Periodic monitoring of your blood values ​​is also advisable if you are taking medication that can affect the bone marrow.
