Eosinophilic granulocytes, also known as eosinophils, are a specific type of white blood cell. The eosinophil distinguishes itself by being particularly active during allergic reactions and parasitic infections. A doctor orders this test when there is a suspicion of an allergic condition, a parasitic infection, or an autoimmune disease. The differential blood count can also help determine a direction in cases of unclear symptoms. What do abnormal values mean?
An elevated value
When the number of eosinophils is higher than normal, it is referred to as eosinophilia. This can indicate an allergic reaction, such as hay fever or other forms of inhalation allergies. Elevated values can also occur with food allergies or asthma.
Another common cause of eosinophilia is infection with parasites, such as roundworms or other intestinal worms. The immune system then deploys extra eosinophils to combat these organisms. This is one of the reasons why doctors also consider parasitic infections in cases of recurring abdominal complaints after travel to tropical areas.
In rarer cases, a significantly elevated number of eosinophils can indicate vasculitis, an inflammation of the blood vessel walls. A well-known example of this is eosinophilic granulomatosis with polyangiitis, also known as EGPA or Churg-Strauss syndrome. In this condition, inflammation occurs in small and medium-sized blood vessels, and the elevated eosinophils are accompanied by other serious symptoms.
A decreased value
A lower-than-normal amount of eosinophils, also known as eosinopenia, is less likely to attract attention but can also be significant. It can occur during acute infections such as influenza, tuberculosis, or HIV. In a severe infection such as sepsis, where the body is overwhelmed by the infection, eosinophils can temporarily drop sharply.
Certain autoimmune diseases and an enlarged spleen are also associated with decreased eosinophil levels. A doctor will never assess this finding in isolation. It forms one piece of a larger puzzle picture, together with other lab values, the medical history, and the patient's complaints.
When is measuring useful?
Measuring eosinophils is useful if you repeatedly suffer from allergic symptoms such as hives, itching, eye irritation, or shortness of breath, and the cause is not yet clear. This test can also provide relevant information if you have recurring gastrointestinal complaints after staying in an area where parasitic infections are more common.
In addition, measurement can be useful in the case of existing autoimmune diseases to monitor disease activity. If vasculitis or other inflammatory conditions are suspected, a doctor will request this as part of a more extensive blood test.
