Monocytes belong to the white blood cells, also known as leukocytes, and form an essential part of the immune system. They are produced in the bone marrow and subsequently circulate through the blood. Although they are relatively few in number in the blood, namely about 2 to 8 percent of all white blood cells, they have a major impact on how our body responds to external threats.
Once monocytes leave the bloodstream and enter tissues, they transform into macrophages. These are larger, specialized cells that literally engulf and destroy foreign particles, bacteria, and cellular debris. In addition to this direct cleanup work, macrophages also play a coordinating role: they transmit signals to other immune cells so that the body can respond quickly and specifically to an infection or damage.
Monocytes also produce substances involved in regulating inflammatory processes. They can either fuel or temper inflammation, depending on what the situation requires. That makes them versatile players in the defense.
What is the measurement used for?
Measuring monocytes provides insight into the state of the immune system. Based on this value, a doctor can assess whether there is an issue with the immune system or if there are indications of an underlying condition. The measurement is part of a differential count, a laboratory test that maps the ratio between the different types of white blood cells.
In addition to monocytes, neutrophils, lymphocytes, basophils, and eosinophils are also measured in a differential count. Each type has its own function, and an abnormality in one or more of these cell types can provide valuable information about what is happening in the body. The differential count is a standard part of a complete blood count.
The analysis can be performed either automatically using a hematology device or manually via microscopic examination. In microscopic examination, a laboratory technician counts the different cell types themselves. Both methods provide a detailed picture of the composition of the blood.
What do abnormal values mean?
Low monocyte counts
A reduced number of monocytes, also known as monocytopenia, can have various causes. Certain infections can temporarily lower the number of monocytes. For example, an infection with the human immunodeficiency virus (HIV) can lead to lower monocyte counts. Parvovirus B19, the causative agent of fifth disease in children, can also have a lowering effect on the number of monocytes in the blood.
In addition, conditions affecting the bone marrow can lead to monocytopenia. In aplastic anemia, the bone marrow produces too few blood cells of all types, including monocytes. The same applies to myelodysplastic syndrome, a group of bone marrow disorders in which blood cell production is disrupted. In these conditions, the numbers of other white and red blood cells often decrease as well.
High monocyte counts
An increased number of monocytes is called monocytosis. This can be a reaction of the body to an infection, particularly bacterial infections such as tuberculosis. Chronic inflammatory diseases can also be accompanied by elevated monocyte counts, including conditions in which the immune system attacks the body itself, the so-called autoimmune diseases.
Blood diseases such as leukemia can also lead to an increase in the number of monocytes in the blood. In that case, the monocytosis is part of a broader disruption of blood cell production. Furthermore, long-term use of corticosteroids, also known as cortisone, can lead to slightly elevated monocyte counts. This is a known side effect of these medications.
When is measuring useful?
A measurement of monocytes is relevant when there are symptoms that may indicate an infection, inflammation, or a disorder of the immune system. Consider persistent fatigue, recurring infections, or unexplained weight loss. Regular measurement can also be useful when monitoring known conditions that affect blood composition.
Monocytes are almost always measured as part of a broader blood test, such as a complete blood count with differential count. In this way, the monocyte value is always assessed in the proper context, together with the other white blood cells and other blood parameters. An isolated monocyte value says less than the overall picture.
It is best to leave the interpretation of the results to a doctor or specialist. Abnormal values are not always a sign of illness, but they do require a careful assessment of your entire health situation.
