What are leukocytes?

Leukocytes, better known as white blood cells, are the guardians of your immune system. They recognize and fight infections, clear away damaged cells, and play a central role in inflammatory responses. Without properly functioning leukocytes, the body is vulnerable to all kinds of ailments.

There are five main types of leukocytes, each with its own task and a recognizable appearance. Neutrophils form the largest group and are the first line of defense against bacterial infections. Lymphocytes are essential for specific defense and produce antibodies, among other things. Monocytes clear away cellular debris and pathogens. Eosinophils play a role in allergic reactions and parasitic infections, while basophils are involved in inflammatory processes.

Assessing the appearance of these cells, known as morphology, provides physicians with insight into how well the immune system functions and whether there is an underlying condition.

Why is leukocyte morphology measured?

Automated blood counts indicate how many leukocytes are present in the blood but provide no information about what those cells look like. Leukocyte morphology fills this gap. A laboratory specialist examines a stained blood smear under a microscope and assesses the shape, nucleus, cytoplasm, and any distinctive structures of the cells.

This examination is performed when blood values ​​deviate inexplicably, for example, due to a white blood cell count that is too high or too low. Morphological assessment is also indispensable when a hematological disorder, such as leukemia or myelodysplastic syndrome, is suspected. Furthermore, the examination can provide insights into identifying the cause of severe infections or assessing medication side effects.

Special findings that the specialist may observe include immature cells, altered granularity in neutrophils, or abnormal lymphocytes. Each of these characteristics has its own clinical significance.

What do abnormal findings mean?

Abnormal morphology can point to various conditions. Hypersegmented neutrophils, cells with six or more nuclear segments, may indicate a deficiency in vitamin B12 or folic acid. Toxic granularity and so-called Döhle bodies in neutrophils are seen in severe infections and sepsis.

Atypical lymphocytes are large, reactive cells that frequently occur in viral infections such as mononucleosis. Blasts, large immature cells with a high nuclear-to-cytoplasmic ratio, may indicate acute leukemia and always require further diagnostic testing. Pelger-Huët anomaly, in which neutrophils have too few nuclear segments, can be caused by both heredity and medication use.

Vacuoles in the cytoplasm, small empty vesicles within the cell, are sometimes observed in sepsis or exposure to toxic substances. All these findings together help the doctor distinguish between a benign reactive change and a potentially malignant blood disorder.

When is it useful to have this measured?

Morphological examination of leukocytes is not a standard blood test that everyone has performed periodically. It is a targeted diagnostic step taken when there are specific questions regarding the function or appearance of white blood cells.

Consider this examination in cases of persistent fatigue combined with an abnormal blood count, unexplained fever or recurring infections, or when a doctor has found an abnormality in the leukocyte count during routine blood tests. Morphological assessment can also yield relevant information in people taking medications that can affect the bone marrow.

The examination is always interpreted in conjunction with other blood values. The findings provide the physician with a richer picture than numbers alone, and thus constitute a valuable addition to routine blood tests for the detection of blood and immune diseases.
